Background
The Cass County Health Department (CCHD), established in 1938, is responsible for a variety of
services that work to prevent illness, promote a healthy lifestyle, and protect the population from
various diseases and health conditions. CCHD serves a population of just over 107,000 and is
divided into nine separate divisions that work towards achieving a healthier Cass County. These
divisions include administration, vital records, environmental health, nursing, public health
preparedness, health promotion and education, medical billing, communicable disease
investigation and the Special Supplemental Nutrition Program for Women, Infants, and Children
(WIC).
The contractor should have expertise in performing community health needs assessments for
local public health departments.
Scope of Services
The contractor will develop and deliver a community health needs assessment which meets
PHAB (Public Health Accreditation Board) standards and follows the MAPP (Mobilizing for
Action through Planning and Partnerships) process.
1. Perform a gaps and weakness scan of current community health needs assessment.
2. Create a detailed implementation plan for community health needs assessment methods;
attend monthly meetings with CCHD staff and identified stakeholders.
3. Implement a series of community health needs assessment methods including:
a. Obtaining secondary data.
b. Hold focus groups across Cass County to identify community assets and barriers
to health.
c. Perform key informant interviews across Cass County to identify community
assets and barriers to health.
d. Development an online survey to gauge health concerns held by community
residents.
4. Complete an analysis of resulting quantitative and qualitative data.
a. Include at least the following ten indicators of health when collecting and
analyzing data:

ii. socioeconomic characteristics
iii. health resource availability
iv. quality of life
v. behavioral risk factors

ix. death, illness, and injury
x. communicable disease
5. Facilitate two community forums to share results of the community health needs
assessment and seek stakeholder feedback.
6. Facilitate prioritization of health issues to be addressed through planning.
7. Create a community health needs assessment report for dissemination which should
include all raw data (qualitative and quantitative; de-identified) and sources, as well as
comparisons of data collected with benchmarks and a summary of findings.
8. Create a PowerPoint presentation from the report for use in community presentations.
General Provisions.
1. Pursuant to RSMo 285.530(1), by its sworn affidavit, the successful contractor
will be required to affirm its enrollment and participation in a federal work
authorization program with respect to the employees working in connection with
the contracted services and that it does not knowingly employ any person who is
an unauthorized alien in connection with the contracted services.
2. A policy of insurance for Commercial General Liability Coverage and
Automobile Liability Coverage shall be provided in the aggregate amount of not
less than $1,000,000 for all claims arising out of a single accident or occurrence
and $500,000 for any one person in a single accident of occurrence.
3. The selection process will be followed by contract negotiation. If negotiations are
unsuccessful, Cass County will proceed with negotiations with the next
qualifying contractor.
4. Cass County reserves the right to accept the proposal that is, in its judgment, the
best and most favorable to the interests of the Cass County.
5. Cass County in its sole discretion reserves further right to:
a. Waive any formality
b. Terminate this RFP, at any time, without penalty
c. Reject any or all proposals received
d. Make any investigations it deems necessary to evaluate the contractor’s
ability to provide the solution CCHD desires

f. Issue amendments in the form of addenda to this RFP prior to the date for
proposal to Cass County which will be provided via the Cass County
website
Proposal Requirements
1) Description of firm with information on previous experience with similar projects
2) Detailed work plan with timeline and milestones
3) Detailed project budget
4) Description of personnel to be assigned to the project, including the proposed project
team.
Funding for this project is contingent on the Cass County Health Department receiving adequate
grant funding.
Proposals are due Friday, March 24, 2023 by 4:00 pm CST and may be submitted either
