Birth and Death Certificates

There are three easy ways to obtain a Missouri birth or death record. Choose the method that best suits your needs.

Walk In

Birth and death certificates are available in our office from 8 a.m. - Noon and 1 p.m. - 4:30 p.m. during the weekdays. Same day service is available to walk-in customers. You must have a photo identification card of some kind (e.g. Driver's license, Work I.D., etc.) or two pieces of mail with your name and current address to obtain the copies. 

Fees

No birth certificates before 1920 and no death certificates before 1980 are available at the Health Department. They may be obtained from:

Mail

You may complete the application and send it to us with payment, a photocopy of your driver’s license, and a self-addressed stamped envelope. For mail-in requests you MUST have the form notarized. Please allow sufficient time for delivery for all birth/death records. 

We no longer accept personal checks.  Payment needs to be in the form of a cashier's check or money order.

Online
